Are you willing to train an hour a day with your dog?


Are your dog(s) up to date on their vaccinations? Can you obtain copies of your dog(s) vaccination records before training?


What are your dog(s) name(s) ?


What are your dog(s) breed(s)??


What are the age(s) of your dog(s)?


How long have you had your dog(s)?


Have you previously had dog(s) before this one? Please describe dog(s) age(s), training level(s), breed(s), personality, and problem(s)?


What goals are you looking for in training your dog(s)?


Do any other adult family members have interactions with the dog(s)? What is the interaction like between them and the dog(s)?


Do you have children? If so what are their age(s)? What is the interaction like between them and the dog(s)?


Have any family members, beside you, been having problems with the dog(s)? If so what types of problems are they having?


Do you have many guests and do they usually greet your dog(s)? Are there any problems with this?


Do your dog(s) have any health problems? If so what are they, and which ones if any cause discomfort?


What  behavioral problems have you seen with your dog? Please list what they are.


If you are having behavioral problems, have you had your veterinarian do a physical check of your dog (blood panels, hip check, arthritis, and more with this problem in mind)?


Please describe all instances where your dog has bitten a human or another animal.  Also, indicate what the bite level was at each occasion (i.e. no mark, scratch, dent, broken skin but shallow, puncture not serious, puncture serious, rip or tear in skin, or more). 


How does your dog(s) react around other strange dogs? How does your dog(s) react around other dogs that they know or are accustomed to? 


Has your dog or dogs received any obedience training at all? If so, what kind of obedience training (positive, motivational, compulsive, Koehler, aversive, combination)? What was the environment like (in-home private/group)? What was your and your dog's reaction to this training?


What kind of exercise does your dog get? Are you also involved in interacting with your dog during exercise? What dog sports (if any) is your dog involved in?


What kind of life experiences (aka socialization) has your dog enjoyed (trips to parks, trips to family members houses, trips to retail stores, trips to veterinarian, vacations with family et)? Generally are there any problems when you go to these places?


What type of environment will your dog be living in? (Apartment, house, yard space et)


What items has your dog destroyed inside or outside the house?

Is your dog housetrained fully (i.e. not pad trained or paper trained)?

Does your dog have any separation anxiety issues?

 

What days and times are best for your lessons? (note in summer we try to keep training in the early daytime hours or starting again in the early evening hours)

Does anyone who will be working with the dog have any physical disabilities themselves?

 

What services are you most interested in?  (Private Training, Day Training, Group Class, Residency also known as Board & Train)
